Uttar Pradesh SIR: As part of the ongoing Special Intensive Revision (SIR) of electoral rolls in Uttar Pradesh, the Chief Electoral Officer on Tuesday announced that citizens applying for voter registration must submit a mandatory declaration form along with Form 6.

Under the updated guidelines, applicants are required to submit their complete details, including their full name and current address with precise spelling, a recent photograph, and a valid mobile number. Furthermore, applicants must provide the details of their parents or grandparents as they appeared in the 2003 final voter list, along with the assembly constituency number, part number, and serial number.

UP SIR: Declaration Form With Form 6 Required For Voter Registration In Uttar Pradesh

- The Election Commission has announced that a declaration form is mandatory with Form 6 for new voter registration under Special Intensive Revision (SIR) in Uttar Pradesh.

- This declaration must include the complete details of the applicant, such as their full name and current address with correct spelling, a recent photograph, and a valid mobile number. Additionally, applicants are required to provide the details of their parents or grandparents from the 2003 final voter list, along with the relevant Assembly constituency, part, and serial numbers.

- While applications that correctly map to these records will proceed without issue, any discrepancies or missing data will lead to a notice from the Electoral Registration Officer (ERO).

- In such cases, applicants will have to appear before the Booth Level Officer along with required documents, such as birth certificates, passports, or educational records, to prove their date and place of birth.

UP SIR: Check List Of Documents For Proof Of Age

If the details provided in the declaration form are not available or do not match the previous 2003 final voter list, a notice will be issued to the applicant. Applicants will be required to submit one of the following documents to verify the date of birth:

- Birth certificate issued by the competent authority
- Aadhaar Card
- PAN Card
- Driving License
- High School/Intermediate certificate mentioning the date of birth
- Indian Passport

In case any of the above required documents are not available, the Election Commission will conduct an on-site verification to confirm the applicant's eligibility.
