The US National Transportation Safety Board (NTSB) has released a chilling CCTV video showing the left engine of a UPS cargo plane separating from the wing just seconds after takeoff who suffered a catastrophic mid-takeoff failure before crashing near Louisville, Kentucky. A total of 15 people died in the fatal accident.

The newly released video shows the left engine and its supporting pylon tearing away from the wing of the McDonnell Douglas MD-11F operating UPS Flight 2976 just seconds after the aircraft lifted off from Louisville Muhammad Ali International Airport on November 4, 2025.

The video clearly shows that the plane lost control before gaining sufficient altitude. According to the flight recorder showed the plane climbed to only around 30 feet before losing control. The aircraft eventually crashed into industrial buildings near the airport, leaving behind a debris trail stretching nearly half a mile.

The crash killed all three crew members on board. At least 12 people working in the industrial area on the ground also died, while more than 20 were injured as per the New York Post. Authorities described the scene as a massive post-impact fire involving aviation fuel and nearby industrial facilities.

The fire, which engulfed the aircraft's fuel and surrounding facilities, was so intense that it engulfed the entire area. This accident became the deadliest in UPS Airlines history.

NTSB investigators have identified engine separation as the primary cause of the accident in their preliminary report. Further investigations are underway into the engine failure, maintenance, and possible technical defects.

A full report based on the examination of the wreckage, black box data, and witness statements may take time. Authorities have advised local residents to stay away from the scene as the risk of toxic fumes from the fuel leak and fire remains.
